Prosthodontic Reconstruction : Core Build-up

If there’s enough coronal tooth structure to yield retention to a core build up, a post will not be necessary. The build up will fill the access cavity and other loss tooth tissue.

For core build-ups, a mechanical retention in addition to dentin bonding system should be placed to achieve maximum retention.

If the remaining tooth structure and the pulpal space support sufficient retention for the buildup, a post will not be needed and should be avoided.

Known advantages of build up without a post:

  • Additional weakening of the root is avoided.
  • Danger of root perforation is minimized.
  • Load transmission to the root is more uniform.
  • Fracture mode is more favorable in case of failure.
  • Salvaging of the tooth is more likely in case of failure.
  • Non-surgical retreatment is facilitated.
  • Costs for materials and treatment time are reduced.

In anterior teeth, the forces act in more horizontal direction and the cross section area is small, thus the insertion of post is necessary depending on the retention of core build up after tooth preparation.

    sir ask ko lng po sna kung pde ipa pasta yung naputol na ngipin ndla po kc ko nung nag basketball ako e kaya balak ko po i pa pasta pde po b yun klhati putol???

    Kapag sumakit na at abot na sa pulp ang bali, hindi na pasta ang dapat gawin. RCT at jacket crown ang dapat. Or bunot at pustiso. Or bunot tapos implants. Itong nasa post na nireply-an mo ay RCT tapos jacket crown.

    It can be restored with composite resin temporarily. If there’s pulp involvement, possible dental treatments are the following: 1) RCT + jacket crown. 2) tooth extraction+denture, or 3) extraction + dental implant.

    Yes. That is possible, but you won’t be able to wear your present denture anymore.
