Affordable Fixed Prostheses

Individual crowns for 11, 21 and 31 and a fixed partial prosthesis to replace 14, 15 and 16. Tooth numbers 13 and 17 were used as the abutment teeth.

For this case the missing teeth are upper first molar, upper second premolar and upper first premolar. A fixed partial denture was fabricated to replace the missing teeth. The cuspid and second molar were used as abutments. They were prepared, 2 mm reduction with shoulder margin on the labial and chamfer on the lingual. Porcelain fused to metal was chosen as the material for this bridge to attain enough structural strength. To balance the occlusion three pontics were used, eventhough esthetically, the space can be filled up by two pontics. Ceramics fused to metal was also used to lower the cost. Individual porcelain fused to metal crowns were also fabricated for the upper lateral incisor, and two upper central incisors. Butt joint was implemented around the margin to hide the reflection of the metal structure labially and of course, to enhance the overall esthetics. TMD in Denture Treatment

Involvement of TMD in prosthodontics is oftentimes ignored. Denture treatment and other dental management usually concentrate on dentition and the surrounding structure setting aside the importance of temporo mandibular joint. We can not get away from the fact that the dental structure is at the interior segment of the mandible and the joint is at the posterior. Any manipulation or correction of the dentition is automatically reflects a certain degree of change at the joint area. Any kind of treatment approach without awareness of the joint’s existence may put the dentist at legal risk.

Causes of Injury in Fixed Partial Denture Tooth Preparation

Causes of Injury in Tooth Prep

Temperature

Considerable heat is generated by friction between a rotary instrument and the surface being prepared. Excessive pressure, higher rotational speeds, and the type, shape and condition of cutting instrument. Feather light touch and water spray should be used.

Chemical Action

The chemical action of some dental materials (bases, restorative resins, solvents and luting agents) can caused pulpal damage.

Bacterial Action

Pulpal damage under restorations has been attributed to bacteria that either were left behind or gained access to the dentin because of microleakage. Antimicrobial agent after tooth preparation and before cementation should be used.

Root canal

Remove only minimal tooth structure. Overenlargement can perforate or weaken the root. The thickness of the remaining dentin is the prime variable in fracture resistance of the root.

Coronal

As much of the coronal tooth structure should be conserved as possible.

Provisional Dental Crowns

Procedure

Tooth preparation for endodontically treated teeth is a 3 stage operation
1. Removal of the root canal filling material to the appropriate depth.
2. Enlargement of the canal.
3. Preparation of the coronal tooth structure.
